New Patient Forms

Here's what you may expect after you've made your New Patient appointment.
1) CONFIRMATION: From Square that your appointment has been made and accepted.
2) FORMS: You will receive an email from Texas Natural Healthcare / Whole Practice with your New Patient forms.
((Forms must be completed BEFORE your appointment or your appointment will be pushed back to a later date.))

Privacy Notice - Upon Request

Going to the doctor can be a little intimidating for some. Your first visit is really about getting to know you and your main health concern(s) and what your overall health care goals are.

Your first visit will have three parts:

Initial Intake Forms & Policy Completion
Health history review, present health issues/concerns and exam
Scheduling any necessary x-rays, lab testing.

Appointment Time: This initial visit can take up to 1-2 hours to complete depending on which New Patient exam you chose and your individual concerns and complexity. Please allow ample time in your schedule.
